Please contact your principal or supervisor as soon as possible if you believe you are being put on unpaid leave in error (i.e. you have uploaded your proof of vaccination, received an approved exemption or accommodation or are still in the appeal process). If you are, in fact, vaccinated, you should report to your school or office on Monday with proof of vaccination and documentation that you uploaded your proof to the DOE portal. Then, call HR Connect at 718-935-4000 after 9 a.m.

If you decide to get vaccinated over the weekend, you may upload your proof of vaccination on the DOE portal and return to your school building on Monday. Please bring your vaccination card or other proof of vaccination with you as well.
